About the Role
As a Licensed Practical Nurse (LPN) at Conviva, you will be an integral part of an interdisciplinary team supporting physicians and Advanced Practice Clinicians (APCs). Your primary focus is ensuring a compassionate, patient-first experience for our senior population. You will play a vital role in assessments, vitals, injections, wound care, and chronic disease management, while ensuring accurate Electronic Health Record (EHR) documentation.
Beyond clinical duties, you will educate patients and their families on medications, prevention strategies, and wellness programs. Working within our collaborative environment, you will contribute to care coordination and discharge planning support, helping to uphold our commitment to infection control, safety, and regulatory standards in all clinical activities.
