About the Role

As a Home Health Licensed Practical Nurse (LPN), you will be the vital link between medical expertise and personal care. This role exists to ensure that patients receive high-quality, individualized treatment plans that promote safety, recovery, and well-being. You will work closely with an interdisciplinary team of nurses, physicians, and therapists to coordinate care that adapts to the unique needs of each patient.

In your day-to-day, you will travel between patient homes to perform comprehensive assessments, administer medications and treatments, and manage complex wound care. Beyond clinical duties, you will serve as an educator, empowering patients and their families with the knowledge to manage chronic conditions, understand medication regimens, and maintain a safe home environment. Your accurate documentation in EMR systems and clear communication with the RN and physician team are essential to maintaining the continuity and quality of care.
