About the Role
You will ignite your life and experience "Living Lifespace" by delivering personalized, meaningful care. In this role, you will partner with an interdisciplinary team to review, plan, coordinate, and evaluate resident care. Your day will involve documenting resident conditions and nursing needs accurately, developing and monitoring nursing assistant assignments, and executing treatments while charting reactions to medications.
You will serve as a vital communication bridge, facilitating updates with families regarding changes in medications or resident status. Additionally, you will coordinate admissions, discharges, and transfers to ensure quality customer service and establish protocols to keep care plans accurate and reflective of each resident's individuality.
