About the Role
In this position, you will provide person-centered nursing care to individuals with diverse needs in home and community-based settings. You will collaborate with an interdisciplinary team to implement care plans, educate individuals and families, and document care accurately. Your work will focus on supporting individuals with behavioral health, brain injury, and other specialized needs while promoting their dignity and independence.
