About the Role
As a Licensed Practical Nurse (LPN), you will be the backbone of our clinical care, delivering direct nursing services that make a tangible difference in residents' lives. This role exists to bridge the gap between medical oversight and daily living support, ensuring that every care plan is executed with precision and empathy.
In this position, you will collaborate closely with Registered Nurses, providers, and interdisciplinary teams to manage individualized care plans. Your day will involve administering medications, monitoring vital signs, performing wound care, and guiding support staff to maintain high-quality standards. Beyond clinical duties, you will serve as an educator for residents and their families, explaining disease management and care routines while fostering an environment of trust and independence.

Key Responsibilities
- check_circleProvide direct, resident-centered nursing care under RN supervision
- check_circleMonitor, record, and report vital signs and changes in resident condition
- check_circlePerform wound care, dressing changes, catheter care, and blood glucose monitoring
- check_circleDocument assessments, care, and interventions accurately in the electronic medical record
- check_circleParticipate in the development and implementation of individualized care plans
- check_circleSupport and guide CNAs and other staff to ensure high-quality care delivery
- check_circleEducate residents and families regarding medications, disease management, and care routines
- check_circleAdhere to infection control, safety, and regulatory guidelines to maintain a safe environment
- check_circleAdminister medications and treatments safely according to provider orders
- check_circleAssist residents with activities of daily living while promoting dignity and independence
- check_circleParticipate in shift reports, interdisciplinary meetings, and quality improvement initiatives
Requirements
- verifiedActive, unrestricted Licensed Practical Nurse (LPN) license in the state of practice
- verifiedCurrent Basic Life Support (BLS) or CPR certification
- verifiedExperience in long-term care, skilled nursing, or similar setting preferred
- verifiedFamiliarity with electronic medical records (EMR) systems preferred
