About the Role
CenterWell Home Health is seeking a Licensed Pharmacist to deliver high-quality, in-home pharmaceutical care. In this field-based role, you will serve as a vital link in the home health ecosystem, performing comprehensive medication reviews and collaborating closely with an interdisciplinary care team to optimize complex drug regimens for patients.
Guided by compassion, integrity, and excellence, your mission extends beyond clinical oversight. You will educate patients and caregivers on safe medication use, adherence, and storage, while actively monitoring for interactions and adverse events. Your work will support safe transitions of care, ensure accurate documentation of clinical outcomes, and contribute to continuous quality improvement within a collaborative, growth-focused environment.

Requirements
- verifiedActive Pharmacist license (state-specific)
- verifiedDoctor of Pharmacy (PharmD) or BS in Pharmacy from an accredited program
- verifiedImmunization certification (where applicable)
- verifiedHome health or clinical pharmacy experience preferred
- verifiedCurrent BLS/CPR certification preferred
