About the Role
Jackson Therapy Partners is seeking a Licensed Clinical Social Worker (LCSW) to deliver high-quality psychosocial assessment, counseling, and discharge planning across diverse healthcare settings nationwide. In this travel and contract role, you will support patients and families facing complex medical and social challenges, coordinate community resources, and collaborate closely with interdisciplinary teams to improve outcomes.
This position is designed for clinicians who thrive in dynamic environments. You will adapt quickly to new clinical settings and populations, providing evidence-based individual and family counseling and crisis intervention. Your work will involve developing and implementing discharge and care plans in collaboration with physicians, nurses, and rehab staff, while advocating for patients navigating complex medical and social systems.
