Community LIFE is a program of all-inclusive care for the elderly, committed to empowering older adults to remain at home while preserving their dignity, independence, and quality of life. Our program brings the region's experts in geriatric medicine and care together to work as a team in specialized Day Centers, helping older adults enjoy the highest quality of life possible. Our professionals are dedicated to keeping older adults independent and in their homes through a wide range of services including medical care, social services, meals, activities, and transportation.
As an Intake Specialist, you will serve as the vital first point of contact for individuals and families interested in enrolling in the Community LIFE program. Your mission is to conduct in-home assessments that identify the unique needs and goals of potential participants and their family caregivers. You will explain the program's benefits and services, answer questions, and complete comprehensive assessments of medical, psycho-social, and environmental conditions.
In this role, you will guide potential enrollees through the clinical and financial eligibility process, working closely with our Medicaid Eligibility Coordinator. You will utilize a customer relations management system to monitor the intake journey and introduce new enrollees to our Interdisciplinary Care Team. A key part of your day involves coordinating in-person orientations prior to enrollment, ensuring a smooth transition into the program.
You will represent the organization professionally to referral sources and families, requiring excellent interviewing, record-keeping, and verbal presentation skills. Success in this position demands empathy and compassion for the aging population, the ability to thrive in a fast-paced environment, and a proactive approach to building trust in the Community LIFE brand.
This position is based in Forest Hills, PA, with extensive local travel required. Mileage is reimbursed at the federal rate. The schedule is Monday through Friday, 7:30 am to 4:00 pm. A valid Pennsylvania Driver's License and a reliable vehicle are required to perform these duties effectively.
