The Housing Supportive Services Program (HSSP) is a specialized initiative designed to provide comprehensive, intensive field-based community mental health services. It specifically targets individuals in Permanent Supportive Housing (PSH) who are formerly homeless and living with Severe Mental Illness (SMI) or Serious Emotional Disturbance (SED). The program operates under The People Concern's model, which emphasizes customer choice, harm reduction, and psycho-social rehabilitation to foster independence and recovery.
As an HSSP Clinician, you will serve as a vital member of a multi-disciplinary team dedicated to ensuring housing stability and community integration for adult members. Your work will blend clinical expertise with field-based engagement, requiring you to conduct psychotherapy in offices, members' homes, and various community settings. The core of this role involves building effective, trusting relationships that facilitate improved physical and mental health while assisting members in navigating daily activities, socialization, and their personal recovery journeys.
You will collaborate closely with each member to develop and update individual service plans, ensuring they reflect their unique goals and progress. Beyond direct clinical care, you will participate in weekly staff meetings, group clinical supervision, and training sessions to maintain high-quality service delivery. The role also includes assisting with program evaluation and participating in an on-call rotation for overnight and weekend emergency interventions, ensuring continuous support for the community.
This position offers a dynamic combination of field and office work at the Magnolia Bloom location in Long Beach, CA. You will regularly travel to different local sites, sometimes walking or driving to meet members. The environment can be busy and at times loud, with exposure to various elements such as heat, dust, and noise. The role requires physical flexibility to sit, stand, bend, and occasionally lift or carry up to 35 lbs.
