This position serves as a vital support system for patients under the IDG Plan of Care, delivering personalized assistance directly within their place of residence. The role is designed to foster a safe, clean, and comfortable living environment while ensuring the patient's well-being is prioritized through compassionate care.
You will act as a bridge between the patient, their family, and the agency's medical team. A key part of your day involves maintaining open, respectful, and supportive communication with everyone involved in the patient's care. You will be responsible for observing and reporting any changes in the patient's condition to ensure continuity of care, while strictly adhering to agency policies and infection control principles.
Beyond direct patient interaction, the role includes essential household support such as light housekeeping, laundry services, errands, and meal preparation, all tailored to the specific Plan of Care. Success in this position requires a genuine interest in the welfare of the ill and elderly, coupled with the ability to follow written and verbal instructions effectively in English.
Candidates must be prepared to undergo a criminal history check as a mandatory step in the selection process. We look for individuals who demonstrate a commitment to professional growth and are willing to attend required in-services to maintain competency.
