As a therapist at CenterWell Home Health, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. Reporting to the Clinical Manager, you will deliver personalized care that focuses on rehabilitation and functional improvement, empowering individuals to overcome physical limitations and enjoy a better quality of life.
In this position, you will evaluate, direct, and provide speech/language pathology services to patients in the home or facility. You will use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing, preventing and minimizing their effects. Your day will involve administering diagnostic tests, providing instruction on alternative communication systems, and offering counsel to patients, families, and healthcare staff. You will also participate in care coordination activities, discharge planning, and the development of treatment plans, ensuring the highest standards of professional conduct and confidentiality.
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
