As a Home Health Speech Language Pathologist, you will evaluate, direct, and provide speech/language pathology services to patients in the home or facility. You will use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing, aiming to prevent, identify, evaluate, and minimize the effects of such conditions. Your day-to-day involves administering diagnostic tests, interpreting therapeutic treatments, and providing instruction to patients, families, and healthcare staff on alternative communication systems when appropriate.
You will actively participate in care coordination and discharge planning, maintaining the highest standards of professional conduct regarding patient confidentiality. Documentation is a key part of the role, requiring you to observe, record, and report changes in patient conditions to supervisors and physicians, as well as prepare written reports of evaluations, visits, and care plans in accordance with company policy.
