As a CenterWell Home Health nurse, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working alongside a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love.

Your day will involve building trust with patients and their families, ensuring that every interaction contributes to their physical, emotional, and social wellness.
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this position, you will be required to be screened for TB. Additionally, as this role involves driving to patient homes, it is part of Humana's driver safety program. Selected candidates must maintain a valid state driver's license and personal vehicle liability insurance in accordance with their residing state's minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
