About the Role

We are seeking a dedicated Home Health Orthopedic Physical Therapist to join our Baylor Program. This unique opportunity is designed for therapists who can dedicate every other Saturday and Sunday to the role. You will focus primarily on Starts of Care (SOCs) within the Northern and Southern Chicagoland areas, including specific territories in Northbrook, Evanston, Aurora, and the Southern Suburbs.

In this position, you will enjoy 1:1 therapist-to-patient ratios, allowing you to focus entirely on delivering exceptional, personalized care. You will work alongside a collaborative team of physicians, nurses, CNAs, OTs, and SLPs to evaluate patient function, determine equipment needs, and manage pain to increase mobility. The role offers flexible scheduling and a variety of assignments to support a healthy work-life balance.
