Mantey Heights Rehabilitation & Care Center, part of Stellar Living in Grand Junction, CO, is seeking a licensed Physical Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate residents and develop individualized treatment plans to improve mobility, function, and quality of life. You will collaborate with an interdisciplinary care team—including nurses, caregivers, physicians, and families—to coordinate resident care and goals. Key Responsibilities
- check_circlePerform comprehensive physical therapy evaluations for residents in assisted living, memory care, and independent living settings
- check_circleDevelop and implement individualized, evidence-based treatment plans to improve mobility, strength, balance, and functional independence
- check_circleProvide therapeutic interventions including exercise, gait training, neuromuscular re-education, and pain-management techniques
- check_circleDocument evaluations, daily treatment notes, progress reports, and discharge summaries in the electronic medical record
- check_circleCollaborate with an interdisciplinary team including nurses, caregivers, physicians, and families to coordinate resident care
- check_circleEducate residents and families on home exercise programs, safety, and strategies to maintain functional gains
- check_circleTrain caregivers on safe transfers, body mechanics, and fall-prevention techniques
- check_circleEnsure adherence to all professional, ethical, and regulatory standards for physical therapy practice
- check_circleRegularly reassess residents' progress and adjust plans of care based on clinical response and goals achieved
Requirements
- verifiedCurrent, unrestricted Physical Therapist (PT) license in the state of Colorado
- verifiedDoctor of Physical Therapy (DPT) or equivalent from an accredited physical therapy program
- verifiedCurrent BLS/CPR certification
- verifiedKnowledge of state and federal regulations for rehab services in long-term care settings

Experience or clinical rotations in skilled nursing, rehab, or geriatric care
