About the Role

We are seeking dedicated Home Health Physical Therapists to join our Baylor Program, specifically focusing on Starts of Care (SOCs) for orthopedic patients. This role is designed for those who can dedicate every other Saturday and Sunday. You will work in a territory based on your zip code, covering Northern and Southern Chicagoland areas, including locations such as Northbrook, Evanston, Wheaton, and the Southern Suburbs.

In this position, you will enjoy 1:1 therapist-to-patient ratios, allowing you to focus on delivering exceptional, personalized care. You will work alongside a team of physicians, nurses, CNAs, OTs, and SLPs to evaluate patient function, determine equipment needs, and manage pain to increase mobility. Your day will involve conducting assessments, documenting progress, and educating patients and families on established goals and self-care strategies.
