About the Role
CenterWell Home Health is seeking a Home Health Clinical Manager to lead clinical operations for our in-home care team. This pivotal role oversees care planning, staff supervision, and quality outcomes for a diverse patient population. You will drive performance improvement, support field clinicians, and foster a culture of compassion, integrity, and excellence in a collaborative, growth-focused environment.
In this position, you will partner with physicians, referral sources, and interdisciplinary teams to optimize care delivery. Your day-to-day involves coordinating admissions, recertifications, and discharges while ensuring accurate and timely OASIS and clinical documentation within EMR systems. You will manage clinical staffing and caseload distribution to ensure efficiency, while actively supporting patient and family education to address concerns and ensure satisfaction.
