About the Role

As a Clinical Manager at CenterWell Home Health, you will lead and support a team of dedicated clinicians who deliver compassionate, high-quality care in the home setting. Reporting to the Branch Director, your role is to guide clinical practice, coordinate patient services, and ensure excellence in every step of the care journey. You will empower patients to achieve their highest level of independence while helping your team thrive in their roles.

In this position, you will oversee clinical operations for the location, ensuring that patient care delivery, staff management, documentation quality, and regulatory compliance are maintained onsite. You will act as a key liaison, coordinating communication among physicians, team members, and caregivers to support care coordination and discharge planning. Beyond management, you will serve as a mentor and educator, guiding clinicians through goal-setting, care planning, and clinical decision-making. You will also be available during operating hours to provide direct clinical support and step in as the Branch Director in their absence when necessary.

Hiring Process

Candidates selected for this role will be required to participate in Humana's Tuberculosis (TB) screening program, as this is a patient-facing position. Additionally, as part of our driver safety program, selected individuals must maintain a valid state driver's license and personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
