We are seeking a self-motivated and detail-oriented Case Management Coordinator to join our Case Management team. This role is designed for individuals who value autonomy and flexibility while coordinating the care of members in the Mid Miami area. The position focuses on evaluating medical needs to facilitate overall wellness, managing a caseload of supportive and medically complex members, and developing proactive courses of action to enhance both short and long-term outcomes.
As a Case Management Coordinator, you will assess, plan, implement, and coordinate all case management activities. Your day will involve determining appropriate services and supports based on members' health needs, including prior authorizations, coordination with primary care physicians and skilled providers, medication reviews, and connecting members with community resources. You will utilize critical thinking to collaborate on care management processes, conduct comprehensive evaluations, and monitor care plan progress to achieve optimal healthcare outcomes.
This is a remote position with a requirement to reside in Miami-Dade County, specifically within the Mid Miami area (zip codes: 33174, 33144, 33134, 33165, 33175, 33184, 33182, 33172, 33126). Training will be conducted remotely via Microsoft Teams for approximately 4-6 weeks. Following training, the role requires approximately 75% travel within the region to visit members in their homes, assisted living facilities, and nursing homes. The standard schedule is Monday through Friday, 8:00 AM to 5:00 PM EST.
