About the Role

Nuvance Health is seeking a Nurse Practitioner to collaborate closely with a Medical Oncologist at Northern Dutchess Hospital in Rhinebeck, NY. Working within a multi-disciplinary, integrated, and disease-site-specific model of care, this position serves as both a provider of advanced nursing care and a Clinical Patient Navigator for medical oncology patients. The role focuses on coordinating and overseeing comprehensive care, conducting health history and physical assessments, and managing treatment infusions and supportive care needs.
