Northwest Journey is a fast-growing organization dedicated to the mental health field, specifically serving children and adolescents. We provide community-based treatment options designed to prevent out-of-home placements and foster family stability. Our mission extends to offering therapeutic support for youth and families transitioning back into the community from such placements.
The Care Coordinator serves as the central force connecting the internal and external treatment teams. In this role, you will obtain, provide, and utilize clinical information to develop comprehensive treatment plans and communicate the client's status to all involved parties. You will act as the primary liaison between the client, their family, schools, and other providers.
As a key member of the team, you will facilitate family participation in the program, communicate with day treatment staff regarding clinical and educational needs, and implement treatment plans. You will also lead interagency meetings to review client progress, manage discharge and aftercare planning, and mediate conflicts when necessary. This position requires building a robust network of community resources to advocate for clients and their families.

Northwest Counseling and Guidance Clinic operates within the Mental Health Care industry, based in Siren, Wisconsin. By working with both the individual child and the family system, the program helps establish sustainable goals and fosters greater stability. Additionally, the clinic partners with county Departments of Human Services to provide Daily Living Skills and Mentoring services for children and families residing in the respective counties.
