Advantmed is seeking a licensed, dependable Nurse Practitioner to conduct structured, in-home wellness and risk-adjustment assessments for Medicare members and other at-risk populations. This PRN role is designed for NPs who value autonomy, professionalism, and accountability, offering a blend of flexibility and consistent visit availability. It is best suited for providers looking for reliable, ongoing supplemental income rather than short-term or ad-hoc work.
As a 1099 Independent Contractor, you will perform focused histories, exams, vitals, medication reviews, and specialized screenings (including Hemoglobin A1C, Spirometry, KED, DRE, and FIT). All assessments are standardized, with comprehensive training, equipment guidance, and EMR support provided from day one. You will educate members on preventive care and chronic disease management while documenting visits to support value-based care and risk adjustment.

We pride ourselves on achieving over 30% in-home assessment completion rates, even within complex populations, thanks to our technology-enabled workflows and strong quality assurance protocols.
Most providers complete 4–5 visits per day based on their availability. You will choose your own weekday schedule, with no evenings, weekends, or on-call requirements. Visits are ready to schedule immediately, and you will have access to dedicated coordinators for scheduling, appointment confirmations, and advanced member support. Travel is expected daily within a 55-mile radius of your assigned county, requiring reliable transportation.
This role requires a minimum commitment of approximately 30 hours per month on an ongoing basis. We value consistency and reliability; therefore, you must maintain ongoing monthly availability, be responsive during onboarding and scheduling, and honor confirmed visits and assignments. You will work independently and professionally in the field, supported by a dedicated coordinator and clinical escalation team.
Candidates must possess an active, unencumbered NP license in the applicable state. While 3+ years of patient care experience and in-home risk-adjustment experience are preferred, we support providers in obtaining additional licensure if needed. The onboarding process includes paid training and standardized workflows to ensure you are ready to begin immediately.
