This position serves as a vital bridge between DaVita's clinical teams and patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD). The primary mission is to facilitate the completion of Care Home Evaluations (CHE) and ensure seamless coordination between Nurse Practitioners, clinics, and patients. Whether conducting in-person visits at nephrology offices or dialysis clinics, or managing high-volume telehealth scheduling for patients unable to travel, this role ensures that care remains accessible and proactive.
Teammates in this role will spend a significant portion of their time on-site at clinics, particularly those focused on ESKD patients. The day-to-day involves welcoming new patients, collecting concerns, and proactively re-engaging high-risk patients to support the nursing team's reachability goals. Beyond direct patient coordination, this role acts as a subject matter expert for the team, often mentoring new hires, leading process improvement projects, and providing coaching to peers.
