About the Role
Hartford Healthcare seeks an Endocrinology Advanced Practice Provider to deliver high-quality, patient-centered care for adults with diabetes and complex hormonal disorders. In this role, you will conduct comprehensive assessments, interpret endocrine labs and imaging, and manage evidence-based treatment plans, including insulin therapies and hormone replacement. You'll educate patients and families, use advanced technology such as pumps and CGMs, and collaborate closely with endocrinologists and a multidisciplinary team.
You will evaluate and manage patients with diabetes, thyroid, and other endocrine disorders in both outpatient and inpatient settings. Your day will involve performing comprehensive histories and physical exams, developing and adjusting treatment plans, and participating in case conferences and quality improvement initiatives.
