About the Role
Hartford Healthcare seeks an Endocrinology Advanced Practice Provider to deliver high-quality, patient-centered care for adults with diabetes and complex hormonal disorders. In this full-time role, you will conduct comprehensive assessments, manage treatment plans, and coordinate care with endocrinologists and a multidisciplinary team.
You will provide evaluation and management of patients in outpatient and occasional inpatient settings, developing individualized treatment plans that include medication management, insulin regimens, and device therapies such as pumps and CGMs. Your work will involve ordering and interpreting laboratory and imaging studies related to thyroid, adrenal, pituitary, and metabolic conditions, while delivering essential education to patients and families on disease management and lifestyle modifications.
