About the Role
We are seeking an Endocrinology Advanced Practice Provider to join our team and deliver specialized care for patients with diabetes and complex endocrine disorders. In this full-time role, you will serve as a vital link in our care continuum, performing assessments, managing treatment plans, and titrating medications and insulin across both inpatient and outpatient settings.
You will work closely with endocrinologists, primary care providers, nursing staff, and allied health professionals to coordinate seamless care for a diverse patient population. This position offers the opportunity to utilize advanced technologies, such as continuous glucose monitors (CGM) and insulin pumps, while contributing to quality improvement and population health initiatives. Your work will directly impact clinical outcomes and help improve the lives of patients navigating chronic disease.
