About the Role
As a Direct Care Worker, you will be the compassionate hands and voice for patients in both home and facility settings. This role exists to support individuals in maintaining their independence while providing essential daily living assistance. You will work closely with nurses and the broader care team to implement individualized care plans, ensuring every patient receives dignified, safe, and personalized attention.
Your day will involve providing hands-on personal care, assisting with mobility, hygiene, and meals, while diligently observing and reporting any changes in a patient's condition to the clinical team. Beyond direct care, you will help maintain a safe and clean environment and collaborate with colleagues to support specific patient goals.
