About the Role
As a Direct Care Worker, you will provide compassionate, hands-on support to patients in both home and facility settings. This role exists to help individuals maintain their independence while receiving the daily living assistance they need. You will be the eyes and ears of the clinical team, observing and reporting changes in patient condition while working closely with nurses and other caregivers to execute individualized care plans.
Your day will involve assisting patients with personal hygiene, mobility, meals, and safety, ensuring a clean and dignified environment at all times. Beyond direct care, you will accurately document patient progress and collaborate with the broader team to support specific patient goals. This position offers opportunities for ongoing training and professional development as you grow within our organization.
Hiring Process
Candidates must be able to pass a background check and health screenings. For roles involving in-home care, a valid driver's license and reliable transportation are required.

Requirements
