About the Role

As a Direct Care Worker, you will be the compassionate heart of our patient-centered environment. This role exists to support patients with their daily living activities, promoting their independence and dignity whether they are in a home or facility setting. You will provide hands-on personal care, assisting with mobility, hygiene, and meals, while serving as the eyes and ears of the clinical team by observing and reporting changes in patient condition.

Your day will involve working closely with nurses and other caregivers to execute individualized care plans. You will ensure a safe, clean, and dignified environment for all patients, accurately documenting care and adhering to safety protocols. Beyond clinical tasks, you will uphold Mosaic Life Care's core values, ensuring every interaction reflects our commitment to patient-centered care.
