About the Role
As a Direct Care Worker, you will be the compassionate heart of our patient-centered environment. Your primary mission is to support patients with daily living activities, helping them maintain their independence and dignity. Whether in a home or facility setting, you will provide hands-on personal care, assisting with mobility, hygiene, and meals while ensuring a safe and clean environment.
This role is vital to the continuity of care. You will work closely with nurses and the broader clinical team to follow individualized care plans, observing and promptly reporting any changes in a patient's condition. By upholding Mosaic Life Care's values, you will not only assist with daily tasks but also contribute to the overall well-being and recovery of those in your care.
