About the Role
As a Direct Care Worker, you will be the compassionate heart of our patient-centered care model. This role exists to support patients in maintaining their independence and dignity while receiving the hands-on assistance they need for daily living. Whether in a home or facility setting, you will serve as a vital link between patients and the clinical team, ensuring that individualized care plans are followed with precision and empathy.
In this position, you will provide essential personal care, assist with mobility and hygiene, and help prepare meals. Your keen observation skills will be crucial as you monitor patient conditions and promptly report any changes to nurses and clinical staff. Beyond direct care, you will collaborate closely with the broader care team to support patient goals, uphold safety protocols, and maintain a clean, dignified environment. Requirements

- verifiedKnowledge of basic infection control and HIPAA standards
