About the Role
As a Direct Care Worker, you will be the compassionate heart of our patient-centered care model. This role exists to support patients in maintaining their independence while receiving the highest quality of daily living assistance. Whether in a home or facility setting, you will provide hands-on personal care, assisting with mobility, hygiene, and meals, all while observing and reporting changes in condition to the clinical team.
Your day will involve working closely with nurses and other caregivers to follow individualized care plans, ensuring that every patient receives dignified, safe, and personalized attention. You will be instrumental in promoting a safe environment and documenting care accurately, serving as a vital link between the patient and the broader clinical team.
