About the Role
As a Direct Care Worker, you will be the compassionate heart of our patient-centered environment. Your primary mission is to support patients with daily living activities, helping them maintain their independence and dignity. Whether in a home or facility setting, you will provide hands-on personal care, assisting with mobility, hygiene, and meals while ensuring a safe and clean environment.
In this role, you will work closely with nurses and the broader care team to implement individualized care plans. Your day will involve observing and promptly reporting changes in patient conditions, documenting care accurately, and collaborating to support specific patient goals. You will uphold Mosaic Life Care's core values, ensuring that every interaction reflects our commitment to patient-centered care.
