About the Role
As a Direct Care Worker, you will be the compassionate heart of our patient-centered environment. This role exists to support patients with daily living activities, promoting their independence and dignity whether they are in a home or facility setting. You will provide hands-on personal care, assisting with mobility, hygiene, and meals, while serving as the eyes and ears of the clinical team by observing and reporting changes in patient condition.
Your day will involve working closely with nurses and other caregivers to execute individualized care plans. You will ensure a safe, clean, and dignified environment for all patients, documenting care accurately and adhering to strict safety protocols. Beyond the clinical tasks, you will uphold Mosaic Life Care's core values, ensuring every interaction reflects our commitment to patient-centered care.
Hiring Process
Candidates must be able to pass a background check and health screenings. A valid driver's license and reliable transportation are required for those providing in-home care. While we welcome applicants with a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) credential, we are committed to finding compassionate individuals ready to grow with us.
