About the Role
We are seeking a compassionate Direct Care Worker to support patients with daily living activities and promote independence within a patient-centered environment. Whether in home or facility settings, you will provide hands-on personal care, assisting with mobility, hygiene, meals, and safety. This role is vital for observing and reporting changes in patient condition to the clinical team, ensuring that individualized care plans are followed effectively.
You will work closely with nurses and other caregivers to support patient goals, upholding our values in every interaction. Your presence helps maintain a safe, clean, and dignified environment for all patients while contributing to their overall well-being.

Requirements
