About the Role
As a Direct Care Worker, you will be the compassionate heart of our patient-centered environment. This role exists to support patients with their daily living activities, promoting their independence and dignity whether they are in a home or facility setting. You will provide hands-on personal care, assisting with mobility, hygiene, and meals, while serving as the eyes and ears of the clinical team by observing and reporting changes in patient condition.
Your day will involve working closely with nurses and other caregivers to execute individualized care plans. From ensuring a safe, clean environment to documenting care accurately, you will uphold Mosaic Life Care's values of compassion and patient-centered care. This position offers a unique opportunity to grow professionally through ongoing training and development while directly contributing to the well-being of those we serve.
