About the Role
We are seeking a compassionate Direct Care Worker to support patients with daily living activities and promote independence within a patient-centered environment. This role is vital for ensuring that every individual receives dignified, personalized attention in both home and facility settings.
In this position, you will provide hands-on personal care, assisting with mobility, hygiene, meals, and safety. You will work closely with nurses and other caregivers to follow individualized care plans, observing and reporting changes in patient condition to the clinical team promptly. Your presence helps create a safe, clean, and supportive environment where patients can thrive.

Key Responsibilities

- check_circleObserve and report changes in patient condition to clinical staff promptly
- check_circleAssist with meals, hydration, and safe feeding as needed
- check_circleDocument care accurately and follow individualized care plans and safety protocols
- check_circleCollaborate with nurses and the care team to support patient goals
- check_circleMonitor vital signs and provide medication reminders as needed
- check_circleEnsure safe transfer and mobility assistance for all patients
- check_circleMaintain infection control standards and HIPAA-compliant communication
- check_circlePerform household support tasks including light cleaning and meal preparation
Requirements
- verifiedCurrent CPR/First Aid certification preferred
- verifiedCertified Nursing Assistant (CNA) or Home Health Aide (HHA) credential preferred
- verifiedValid driver's license and reliable transportation if providing in-home care
- verifiedAbility to pass background check and health screenings
- verifiedKnowledge of basic infection control and HIPAA standards
Nice to Have
