About the Role

As a Direct Care Worker, you will serve as a vital link in our patient-centered care model, supporting individuals with their daily living activities to promote independence and dignity. Whether in home or facility settings, your role is to provide hands-on personal care, assist with mobility and hygiene, and ensure a safe, clean environment for every patient.

You will work closely with nurses and the broader clinical team to implement individualized care plans. Your day will involve observing and promptly reporting changes in patient conditions, assisting with meals and hydration, and documenting care accurately. This position is ideal for someone who values empathy and wants to be part of a team that upholds the highest standards of patient-centered care.
