About the Role
As a Direct Care Worker, you will be the compassionate heart of our care team, supporting patients with daily living activities to promote their independence and dignity. Whether in a home or facility setting, your role is to provide hands-on personal care, assist with mobility and hygiene, and ensure a safe, clean environment. You will work closely with nurses and other caregivers to follow individualized care plans, observing and reporting changes in patient condition to the clinical team promptly. This position is vital for maintaining the well-being of our patients while upholding Mosaic Life Care's core values.
Hiring Process
Candidates selected for an interview will be required to pass a background check and health screenings. Please note that a valid driver's license and reliable transportation are necessary for roles involving in-home care.

Requirements
