About the Role
We are seeking a compassionate Direct Care Worker to support patients with daily living activities and promote independence within a patient-centered environment. Whether in home or facility settings, you will provide hands-on personal care, assisting with mobility, hygiene, meals, and safety. This role is vital for observing and reporting changes in patient condition to the clinical team, ensuring that care remains responsive and effective.
You will work closely with nurses and other caregivers to follow individualized care plans, upholding Mosaic Life Care values of compassion, teamwork, and patient-centered care. Your efforts will help create a safe, clean, and dignified environment for all patients while supporting their unique goals.
Hiring Process
Candidates must be able to pass a background check and health screenings. A valid driver's license and reliable transportation are required for those providing in-home care.
