Customer Service Liaison

About the Role

We are seeking a compassionate and experienced Customer Service Liaison to serve as the vital bridge between Heart to Heart and the broader healthcare community. This role is designed to promote our hospice presence within assisted living facilities, nursing homes, home health agencies, and hospitals. You will be the face of our organization, ensuring that patients and families receive a high level of satisfaction while coordinating care among various healthcare professionals.

In this position, you will foster effective working relationships with staff, facilities, the lay and professional public, the Board of Directors, and the Medical Director. Your day-to-day involves communicating regularly with patients and their families to ensure all needs are met, identifying any issues or dissatisfaction, and facilitating quick resolutions in collaboration with the clinical team and management. Additionally, you will coordinate and provide educational in-services for families and facilities, working closely with agency leadership to meet growth and development targets. Flexibility is key, as you may need to provide after-hours and weekend consultations to support referral sources, patients, and families.
