About the Role
As a Contracted Direct Care Worker, you will play a vital role in supporting patients with their daily living tasks in home or facility settings. This position is designed for individuals who are passionate about making a meaningful impact on the lives of others every day.
You will collaborate closely with an interdisciplinary healthcare team to deliver care that aligns with individualized care plans. Your day-to-day involves providing personal care, assisting with mobility and transfers, and offering companionship and emotional support to patients and their families. By following strict safety protocols and maintaining accurate documentation, you help ensure a safe and supportive environment for those we serve.
This contracted role offers the flexibility to tailor your schedule while providing opportunities for skills growth. Whether you are assisting with bathing, grooming, and meal preparation or monitoring patient conditions, your work directly contributes to the well-being of our community.
Hiring Process
Interested candidates must be able to pass a background check and complete health and TB screenings. Please note that a valid driver's license and reliable transportation are required for in-home care positions.

Requirements
