About the Role
We are seeking a compassionate Contracted Direct Care Worker to support patients with daily living tasks and promote independence in home or facility settings. This role is designed for individuals who thrive in a collaborative environment, working alongside an interdisciplinary healthcare team to deliver person-centered care.
As a Direct Care Worker, you will be the vital link between clinical oversight and the patient's daily life. Your day will involve providing personal care, assisting with mobility, and offering companionship while strictly adhering to individualized care plans. Whether in a home or facility setting, you will ensure patient safety and comfort while maintaining the highest standards of privacy and professionalism.
Hiring Process
Interested candidates should submit their application including proof of current CPR/First Aid certification and relevant credentials (CNA or HHA preferred). All selected candidates must successfully pass a background check and health/TB screening prior to starting.
