As a Community Support Specialist – Youth, you will provide personalized, in-home and community-based support, helping young people and their caregivers build resilience, navigate challenges, and create lasting change. You will deliver strengths-based, holistic services to youth and families based on individualized Wellness Plans. Your day will involve supporting family engagement in the treatment process, providing coaching and skill-building, and connecting clients with community resources. You will collaborate with school staff, healthcare providers, and other child-serving systems, offering crisis support when needed and assisting in developing safety plans.
Flexibility is key; you will travel within the community to meet with clients and families in their homes, and your schedule may include some evenings and weekends to meet client needs. You'll be part of a team that balances teamwork with independence, communicating regularly with the client's integrated care team and documenting services accurately.
Both experienced professionals and those new to the human services field are welcome to apply. We are looking for individuals who bring compassion, patience, and energy to every interaction. You should be comfortable working in the field, building trust with clients and partners, and being adaptable and solution-focused.
