The Collaborating Physician plays a pivotal role in our integrated care model, working closely with Advanced Practice Nurses (APNs) who conduct in-home and telehealth wellness assessments for patients designated by HarmonyCares Medical Group. While APNs do not hold prescriptive authority or treat patients independently, they rely on the Collaborating Physician for critical clinical oversight.
In this position, you will serve as the medical authority for your assigned team, ensuring high-quality care delivery. Your day-to-day involves being available for consultation—either via telecommunication or in-person—to address patient assessment questions raised by the APN. You will also conduct regular annual chart reviews and collaborate with the APN to ensure strict adherence to policies, procedures, state laws, and quality standards. A key part of your responsibility is maintaining accountability for outstanding customer service to both internal and external stakeholders.
