We are launching the TZield Program, a multidisciplinary clinic dedicated to the management of early-stage Type 1 Diabetes. This program is designed to provide a supportive team environment that fosters professional growth and development for our staff while delivering comprehensive care to patients and families.
We are seeking a full-time Clinical Social Worker to join the TZield Program team. In this role, you will provide a range of clinical social work services, conducting psychosocial assessments, crisis intervention, and individual and family counseling. Your work will extend to potentially leading group psychoeducational interventions, delivered both in person and via telehealth.
You will serve as a vital link for patients and families, collaborating with Patient Navigators to help them gain access to essential services and resources both within and outside the BCH system. A core part of your day will involve consulting with a multidisciplinary team to manage complex psychosocial issues, including coping with medical diagnoses, treatment regimen challenges, family distress, parenting concerns, trauma, and child protection or domestic violence situations.
Beyond direct patient care, you will have the opportunity to shape the program's future by developing new initiatives to support our community and contributing to research and Quality Improvement efforts within the TZield Program.
This position is based at our main location with occasional travel required to our satellite location in Waltham. New hires are eligible for a $5,000 sign-on bonus.
