About the Role
Tenet South Carolina is seeking a Clinical Pharmacist to join our team in Fort Mill, South Carolina. In this decentralized, unit-based position, you will serve as a valued member of the patient care team, working directly within clinical areas to optimize medication therapies.
Your day will involve monitoring drug selection, dosage, frequency, and potential interactions, while ensuring accurate medication reconciliation and discharge planning. You will act as a key resource for drug information and education for physicians, nurses, and patients. A significant part of your role includes rounding with Intensivists and Hospitalists to actively recommend and implement therapy changes. You will also manage specific dosing and monitoring protocols for aminoglycosides, vancomycin, and Coumadin, as well as write TPN orders and handle IV to PO conversions based on renal function.
