As a Clinical Pharmacist at Monument Health, you will play a vital role in delivering comprehensive medication management. Your work ensures that care is seamlessly coordinated among providers and caregivers as patients transition between various care settings. You will act as a key resource in your specialty area, leading the development and maintenance of pharmacy services while adhering to accepted hospital standards and federal and state pharmacy laws.
In this PRN role, you will identify and resolve medication-related problems, monitor patient responses to therapy, and provide accurate drug information to both the multi-disciplinary team and patients. You will also participate in quality improvement projects and manage specific pharmacy protocols, such as therapeutic interchange and renal dosing.
