As a Clinical Nurse in our home health setting, you will provide competent clinical nursing care consistent with professional standards. Reporting to the Nurse Manager or Director, you will be responsible for coordinating and delivering patient care utilizing the nursing process. This role is designed for those who wish to collaborate with patients and families to prepare and update plans of care, addressing complex physical, psychosocial, educational, and safety needs. You will work within an interdisciplinary team to evaluate patient outcomes, promote continuity of care, and advocate for patients and their families.
