About the Role
As a Clinical Care Manager (RN), you are at the center of that impact. This role is designed for a strong clinician who thrives in ownership, collaboration, and a fast-paced environment. You will serve as the clinical quarterback, connecting the dots across patients, providers, and teams to lead care that truly makes a difference.
You will manage your day with autonomy while leveraging strong leadership and clear processes. Your decisions will directly influence patient outcomes, experience, and the overall quality of care. Whether coordinating complex cases, leading case conferences, or supporting in-home visits, you will ensure care is coordinated, compliant, and aligned with patient needs.

Key Responsibilities
- check_circleCoordinate and integrate patient care across clinicians, caregivers, and providers
- check_circleManage physician orders, documentation, and workflows within the EMR
- check_circlePartner with Intake to support timely admissions and reduce referral delays
- check_circleConduct ongoing patient assessments and ensure plan-of-care alignment
- check_circleLead case conferences, recertifications, and discharge planning
- check_circleEnsure compliance with clinical, state, and federal regulations
- check_circleServe as a key liaison between physicians, patients, families, and internal teams
- check_circleSupport care delivery through in-home visits as needed
- check_circleReview clinical reports and take appropriate action regarding occurrences and infection control
Requirements
- verifiedActive RN license in state of practice
- verifiedGraduate of an accredited nursing program (BSN preferred)
- verifiedHome Health experience strongly preferred
- verifiedExperience in care coordination, case management, or clinical leadership preferred
Nice to Have
BSN degree, Home Health experience, experience in care coordination, case management, or clinical leadership.
